Dermal fillers are safe, effective, and long-lasting treatments for facial contouring, facial lines and grooves, and lip enhancement by restoring volume. The procedure is quick, and there is no significant recovery time. While there are different types of dermal fillers, Dr. Phan prefers hyaluronic acid dermal fillers for their effective and safety profile, as well as for its reversibility with hyaluronidase enzyme if the result is not to one's expectation. 

Where can dermal fillers be injected?
Dermal fillers can be injected almost any where on the face. 

For facial contouring, Dr. Phan likes to place Voluma along the cheek bone, jaw and brow bone to enhance facial bone structures. She also likes to use Voluma to build the nasal bridge and contour the nose. This is known as liquid rhinoplasty. Voluma can also be placed in the temples where there are volume loss and thus scalloping of the temple. Beside Voluma, Restylane Lyft may be used in these areas. 

For facial grooves and lines, Dr. Phan uses Restylane, Restylane Lyft, Restylane Defyne, Restylane Refyne, Volbella, Vollure, or Juvederm Ultra or Ultra Plus, depending on the location and severity. In the tear trough (nasojugular) grooves, where the skin is thin, Dr. Phan likes to use Volbella for mild and Restylane or Vollure for more significant hollowing. She also likes to use Volbella in very fine lines, Juvederm Ultra for mild grooves, and Restylane, Juvederm Ultra Plus or Vollure for more moderate grooves. For deep grooves, Dr. Phan places Restylane Lyft in the deeper dermis and layers one of the other dermal fillers on top of the Restylane Lyft. For areas where there is more movement, such as around the mouth, Dr. Phan uses Restylane Refyne for finer lines and Defyne for deeper lines. 

For lip enhancement, Dr. Phan uses Volbella and Restylane Refyne to inflate thin lips and Juvederm Ultra for volumization as well as contour enhancement of the lips. Ideally, the center of the upper lip, called the Cupid's bow, dips down between two columns that extend up toward the nose; and the lower lip is pouty in the center with two pillows. How are fillers administered?
Topical numbing cream is applied for 20-30 minutes, then the product is carefully injected under the skin using an ultra-fine needle. After the injections, there may be gentle massaging of the skin to optimize contouring. To increase the comfort of the procedure, most fillers are pre-mixed with lidocaine, a numbing agent, in addition to the topical numbing cream.

Is there any post-treatment care needed?
After the treatment, the area may be pink with tiny injection marks. Patients may have differing degrees of swelling depending on the site treated and the amount of products used. Bruising may occur. Bruises generally last few days to a week and vary in severity and duration between different treatment sessions. Patients with thin skin or tendency to bruise may use Arnica three days before and a week after injection. Patient may want to sleep on their back that night if possible. Make-up can be applied immediately as needed. If the area around the mouth is treated, dental work should be delayed 2-3 weeks to limit stretching of that area.

When will I see the results?
The results are immediate. However, it typically takes couple to few days to see the full improvement as any redness and swelling resolve.

How do I prepare for treatment?
Patients should avoid taking aspirin, NSAIDS (Ibuprofen, Advil, Motrin), St. John’s Wort, fish oil, flax seed and high doses of Vitamin E supplements for 10 days prior to and 1 day after treatment. This will help to minimize bruising. Patients who have a history of cold sores should inform the doctor of this history before injections around the mouth. Prophylactic anti-viral medication may be prescribed three days before and one week after the injection to avoid recurrence of the cold sores.

How long do fillers last?
Duration depends on the area treated and the type of filler. Restylane typically lasts 6-9 months. Juvederm and Volbella may last 12 months. Vollure lasts up to 18 months. Voluma can last 24 months. With each additional treatment, the length of the duration increases. Which filler to use depends on the area treated. This is when the art and the expertise of the doctor are paramount.
